Question: I am losing hair from my body as well as my head – my scalp hair has been thinning for a while then I started to see a couple of bald patches on top. Now I notice less hair on my arms and chest too. Male Pattern Baldness runs in my family but I don’t think anyone of them has experienced body hair loss, just their heads. Does it cause body hair loss too sometimes? Can you give me advice for treatments?

Answer: Hi, Basil. We would need to assess your scalp and take a few more details in order to provide you with a confirmed diagnosis. However, we can give you the following generalised advice regarding your query.

From what you say, it is possible you may have two separate hair loss conditions.

You mention you have had on-going hair thinning for a while and this is a key sign of the hereditary condition Male Pattern Baldness, which you have already advised runs in your family. This causes thinning hair to the top of the scalp only, from the hairline to your crown, and can also present as a receding hairline. It is not known to affect body hair, nor other areas of the scalp.

Secondly, you note bald patches developing on your scalp plus further hair loss from your body. These symptoms suggest autoimmune-related shedding from a form of Alopecia Areata. This is a condition whereby the body mistakenly attacks its own hair follicles, which can be temporary or permanent, but its duration is currently impossible to predict.

The Alopecia Areata phenotype which is known to cause shedding of body hair is Alopecia Universalis. This tends to cause total baldness from head to toe, so it is extremely important you see a professional trichologist to get an accurate diagnosis, given you do not mention your scalp or body hair falling out completely.

Male Pattern Baldness is a genetic condition where hair loss appears gradually as the result of a biological sensitivity to the hormone dihydrotestosterone (DHT), whereas Alopecia Areata – and Alopecia Universalis – are autoimmune disorders which come on suddenly.

Both Male Pattern Hair Loss and Alopecia Areata can present simultaneously, and whilst Belgravia offers hairloss solutions for the former, we cannot treat Alopecia Areata when it affects the body. This generally requires hospital-based therapies, such as steroid injections or immunotherapy, and your doctor should be able to refer you.

With regards the genetic thinning, there are two clinically-proven, MHRA-licensed and FDA-approved male pattern hair loss treatments available – one oral and one topical. These can be used individually or, as our hair loss specialists advise, together, and additional hair growth supporting products can be used alongside this pharmaceutical approach.

However, should you be diagnosed with Alopecia Universalis, it is unlikely to be worthwhile using hairloss treatment for Male Pattern Baldness as the area of your scalp affected by the genetic condition may become hairless as a result of this condition. This is not something we can diagnose or predict without a thorough consultation.

There are other causes of body hair loss which should also be considered; for example, body hair – specifically leg hair – is known to get thinner with age, and thyroid problems have been shown to lead to hairloss from the body as well as the scalp.

Hair growth can also be negatively impacted by certain skin conditions and as a side effect of some medications, too.

To determine if hair loss treatment is appropriate in your case, and – if so – the optimal selection of hair loss products for your specific condition(s), we reiterate the need for a professional consultation, ideally in-person if possible.

Also, please be aware that due to the Christmas period postal service, we are only able to send out treatments on the following dates so if you are not scheduled for your next delivery but think you may run out over the holidays, please let your Treatment Advisor know as soon as possible. We recommend you order your medications at least one week in advance of when you need them due to potential postal service delays over the Christmas period.

We recommend ordering any medication you may need for over the holiday period by no later than Friday 13th December 2019 for those being sent within the UK, and by no later than Friday 6th December 2019 for those being sent internationally.

The last date for treatments to be sent out to clients before Christmas is Friday 20th December 2019. Following the UK Bank Holidays, postage will resume on 27th, 30th and 31st December 2019. There will be no postage after that until 2nd January 2020 when our normal service resumes.

Please note that due to the Royal Mail Christmas service and seasonal delays packages sent during this time may take longer than normal to be delivered. This includes ‘Next Day’ and ‘Special Delivery’ services which we have been informed can take up to 48 hours during the festive season. Please be aware that, because of this, packages sent between 20th and 30th December 2019 may not arrive until 2nd January 2020.

I Need Help with Thin Hair from Hair Breakage

Question: Hello, for the past 10 months my hair has become dry, frizzy, breaking off and noticeably thinning. I saw a trichologist and she has said I have Telogen Effluvium. However, I am querying this diagnosis as I do not and have not experienced excessive hair shedding. Can you still get Telogen Effluvium without having excess hair shedding? Does this condition cause the hair to become dry, brittle, frizzy and unable to hold moisture? She has also recommended a 6 month course of supplements to treat this condition. Will supplements reverse Telogen Effluvium???

Answer: Hi, Lou. Telogen Effluvium (TE) is a disturbance in the hair growth cycle which makes many strong, actively-growing hairs – known as ‘anagen hairs’ – prematurely enter the resting – ‘telogen’ – phase and fall out.

The main feature of this hair loss condition is diffuse hair shedding from all over the scalp, with telogen bulbs.

It is not typical in cases of TE for there to be hair breakage – where the hair snaps along the shaft – without hair loss from the root. This may happen due to poor hair quality or over-processing, however, and may explain the concerns you have regarding dry hair, brittleness and frizzy hair; damaged hair where there is breakage can often present with these qualities. Additionally, some rare hair shaft disorders can also cause breakage to occur.

Whilst not intended to replace a balanced diet, targeted food supplements, such as Hair Vitalics, or ones which address any specific nutrients you are lacking, may be useful.

If you are unsure whether or not you have any nutritional deficiencies, you can check this by arranging a blood test via your GP.

If your trichologist diagnosed you with Telogen Effluvium, we would assume they saw a number of telogen hairs during your scalp examination. It is possible to have both TE and hair breakage at the same time, though it is unlikely for TE to present without any noticeable hair fall.

The phrase ‘damned with faint praise’ comes to mind when considering the response to the latest study into platelet-rich plasma (PRP) therapy as a potential treatment for Female Pattern Hair Loss.

Whilst the procedure is certainly popular among some men and women seeking non-surgical hair loss solutions, to date it has not shown strong proof of efficacy in any long-term or large scale trials. This is why we do not currently offer PRP at Belgravia.

A team from the Mount Sinai School of Medicine in America carried out a small-scale trial, investigating the effects of PRP scalp injections on 30 women with genetic hair loss.

PRP

The research was led by Dr. Matthew J. Lin, who announced details at the American Society for Dermatologic Surgery’s annual general meeting in October 2019, said of the results, “PRP given at monthly intervals to women with androgenetic alopecia is a reasonable treatment.”

Hair density and diameter changes

PRP involves blood being taken from a patient, spinning it in a centrifuge to separate the platelet-dense part, rich in natural growth factors, so that it can be removed and injected back into the body; in this case, into various areas across the top of the scalp – the area affected by thinning hair caused by genetic hairloss.

In some cases, supplementary additives can also be included in the formula which is then injected back into the patient. This did not happen in the Mount Sinai trial, however.

The prospective, controlled clinical trial involved each of the 30 participants being randomly and blindly assigned scalp injections of either PRP or saline solution (a placebo for control purposes).

There were three sessions in total; the first took place at the start – week 0, or baseline – then there were two further sessions at four-week intervals – week 4 and week 8.

Of the 30 women taking part, 28 successfully completed the necessary treatment. Each one had their hair density – the number of hairs per square centimetre – and hair ‘caliber’ – the diameter of a strand – (mm), recorded at weeks 8 and 24.

Compared to the placebo group (-26.7 hairs/cm2), an improvement of +71.1 hairs/cm2 mean hair density was observed at week 8 in those given PRP. At week 24 the PRP group recorded a mean hair density of +105.9 hairs/cm2 versus -52.4 hairs/cm2, p<0.01 for the placebo users.

Mean hair diameter measurements also increased slightly for the PRP group at week 8, (+0.0043 mm vs. -0.0034 mm, p<0.01) and week 24 (+0.0053 vs. -0.0060 mm, p<0.01).

PRP patients also came out on top in the additional blind global photographic assessments, used to determine overall hair growth improvements. These showed week 24 improvements of 57 per cent from baseline for the PRP group, compared to 7 per cent of saline patients.

No serious or long-term side effects were noted, though approximately 50 per cent of the PRP group experienced mild headaches and scalp tightness for up to 24 hours.

During the presentation, Dr. Lin explained that, due to the small number of participants with limited diversity (most were Caucasian or Hispanic) and short duration, “Further studies are needed with longer follow-up to determine the best way to integrate this treatment into hair loss therapy.”

It is worth noting that this study was funded by Eclipse Aesthetics, LLC – a medical technologies company and PRP solutions provider.

Treating female pattern hair loss

There are MHRA-licensed, FDA-approved and FDA-cleared women’s hair loss treatments currently available to help regrow hair in cases of Female Pattern Hair Loss.

Topical applications of the clinically-proven medication high strength minoxidil have been shown to accelerate hair growth for many women, whilst additional hair growth supporting products, such as the LaserBand devices, can further stimulate the hair follicles.

Topping up this pharmaceutical and low-level laser therapy approach can be augmented with in-clinic therapy sessions and/or one-a-day food supplements, such as Hair Vitalics for Women, to help maintain normal healthy hair growth.

As this hereditary hair loss condition is both permanent and progressive, those wishing to prevent further hair loss are advised to have a consultation with a dedicated hair specialist at their earliest opportunity.
