‘Hair Loss Treatment Advice Needed For Transgender Male to Female’

Name: Mandy

Question: I am a transgendered male and I would like to know if oestrogen treatment would help me with hair loss?

Answer: Hi Mandy. Oestrogen treatment will not help or stop genetic hair loss, I’m afraid. Only clinically-proven hair loss treatments will counteract the effect of DHT, the hormone derivative which causes the hair to thin.

We have a number of transgender clients who have shown promising regrowth results from using minoxidil. This is a topical product, suitable for both men and women, and is applied directly to the scalp daily to stimulate regrowth.  Some also take finasteride 1mg, an oral male hair loss treatment, which is still suitable for women who are fully transgendered, although it is not recommended for those undergoing female-to-male transitions.

This response is based upon the premise that you are experiencing male pattern baldness. The reason I say ‘male’ is because, despite being transgender, it is important that the diagnosis is linked to your birth gender as male and female pattern hair loss display in different ways.

If you are born male, the signs of hair loss will tend to take the form of thinning hair in defined areas around the top of your head – such as a thinning crown or a receding hairline – which may eventually lead to baldness. For those born female, the key sign is a general thinning of the hair around the top of the head, often affecting the temples and making the parting appear wider. This rarely leads to true baldness, which is where the skin takes on a shiny appearance due to the hair follicles dying and scarring over, rending any treatment redundant.

If you are unsure as to whether your hairloss is of the genetic kind or whether it may be caused by another reason, such as being a side effect of medication or surgery, or caused by stress, we recommend you consult a hair loss specialist for a personal assessment. This can be done in person or online, if you are unable to visit one of our Central London clinics.
